***The Health and Safety of your Dog is Our Top Priority***
​
-We reserve the right to refuse a dog at check in for any reason including without limitation, if it appears your dog is sick, injured or in pain or that its behavior could jeopardize the health and/or safety of other dogs and our staff.
-We reserve the right to refuse service to you for any reason.
-If your dog(s) exhibits behaviors that could jeopardize the health and/or safety of other dogs and staff, you and/or your emergency contact must immediately pick up your dog(s).
-If your dog(s) is ill or becomes ill and this could jeopardize the health and/or safety of other dogs and staff you and/or your emergency contact must immediately pick up your dog(s).
-Your emergency contact must be eighteen years or older and be able to make decisions for your dog. Please make sure they are available to pick up your dog at a moment's notice.
-You must have a valid credit card on file in order for us to care for your dog.
1. Prior to entry: Your dog, regardless of age, must be up to date on all required vaccinations and you have provided us with the proper documentation.
-We require Rabies, Distemper/Parvo and Bordetella
-We do not accept dogs who for any reason cannot receive these vaccinations
-We do not accept vaccination records that have been administered by someone other than a licensed veterinarian
-We do not accept Titer's tests
-We do not administer vaccinations
-Vaccinations MUST be administered by a licensed veterinarian at least 72 hours prior to check in
2. Dogs over the age of 12 months MUST be neutered/spayed. NO EXCEPTIONS.
- We do not accept dogs in heat or about to go into heat in our facility. If your dog goes into heat while in our care, there is an inconvenience fee of $250 and your dog must be picked up immediately.
-If your male dog under the age of 12 months displays behaviors showing dominance aggression, you must pick up your dog immediately.
3. Dogs must complete a temperament test prior to any daycare reservations.
-This includes new clients and prior clients who have not been in our facility for more than 6 months.

4. We reserve the right to use photographs, video footage and any other media of anyone or thing that is in our facility. These images may be used for all forms of marketing materials. We have live security video and web feeds available to the public.

5. Medications: All mediations must be in a container with a current prescription label from your veterinarian. Supplement not labeled for animals or not prescribed by a veterinarian will only be given with your permission and you assume responsibility for their use.
-We do not administer injections of any kind. NO Needles On Site.

6. All payments, deposits, and packages are NONREFUNDABLE. WE DO NOT ISSUE REFUNDS OF ANY KIND. NO EXCEPTIONS. Even if, in the worst-case scenario, your dog displays behaviors that jeopardize our staff and client's health and/or safety and are asked to be removed from Unleashed, all payments, deposits and packages are NONREFUNDABLE.
-Please bring your own dog food in a sealed container or Ziplock bag.
-Refrigerated food must be pre-measured and in a sealed container or Ziplock bag.
-We do not have house food.
-Personal Items: Please do not bring items with your dog that are invaluable or irreplaceable.
-Unleashed is NOT RESPONSIBLE for loss or damage to personal items or toys left with your dog, or any items that may be left behind at pickup. It is your responsibility to make sure you receive your dog's items, leashes etc. that you brought with your dog.
-If your dog destroys any property of Unleashed during their stay, you acknowledge and agree that you are responsible for charges incurred to replace such items.

7. You understand that it is possible for a dog to become ill even if vaccinated. You assume all risks and accept responsibility for all costs and treatments.

8. Please be advised: Your dog will comingle in a free play style with other dogs. Infectious diseases, minor injuries, dog bites, dog scratches, teeth grazing, etc. may occur even when vaccinated dogs interact with each other. You accept responsibility for all costs and treatments.

9. You acknowledge and agree that in the unlikely event another dog injures your dog you release Unleashed and its employees from any liability for such injury. Furthermore, if your dog injures another dog, you will solely be responsible for any injury to the other dog as well as your own dog and you release Unleashed and its employees from any liability for such injury.

10. Cancellation Policy: If you cancel for any reason, at any time, please know we do NOT ISSUE REFUNDS. If you choose to prepay, and cancel, we will no tissue a refund. There are no fees or time limits on a cancellation.

11. Daycare is open Monday-Friday 830-6. We do not offer boarding.
-Dogs not picked up by 6 pm will automatically incur a $25 after hours fee.

12. Abandonment: If you are unable to pick up your dog at checkout, we will make every effort to contact both you and your emergency contact, if unavailable we will keep your pet at YOUR expense for a maximum of three days.
-Each day your pet is with us, you will be charged our normal fee as well as an inconvenience fee of $250.
-If we still have not received any information from you or your emergency contact, we will deliver your dog, at our discretion, to animal control, the humane society, or rehome your pet ourselves.
-If the dog is deemed abandoned by local, state or federal law you release Unleashed from all further liabilities and responsibilities for your dog. You shall remain liable to Unleashed for all unpaid charges including court costs, collection fees and reasonable attorney's fees incurred in the collection of the charges.
13. Payment is due in full at original check out date and time. We accept major credit cards and cash. Checks are only accepted from established clients. If your check is returned, there is a $40 inconvenience fee on top of the bank fees incurred. You shall remain liable to Unleashed for all unpaid charges including court costs, collection fees and reasonable attorney's fees incurred in the collection of charges.

14. Drop off/Pick up Services: This service is a first come/first served.
-Please understand that we may not be available at all times. Drop off/Pick Up must be scheduled in advance.
-This service is offered at a rate of $25.00.

15. In Home Visit Service: You must schedule an in-home visit consultation before your first home visit. -Expectations must be set at that time.
-Each home visit service must be prepaid with a nonrefundable deposit in advance.
-If you cancel or do not show, we do not offer refunds.
-This service is offered at a rate of $25.00 per half hour.
-Please note: We do not stay overnight in your home with your dog.

15. Services outside of our normal business hours: $25 additional charge for any services given outside of our normal business hours.
-Saturday Daycare: We occasionally offer daycare on Saturday. This is offered at our discretion.
-All Daycare rules apply.
16. Boarding Service: Boarding is offered only to our established clients. We only allow two dogs maximum at a time. There is a $99 nonrefundable deposit to create a boarding reservation. Boarding is done in the owner's home in a family environment. All daycare policies apply. The dogs that the owner CHOOSES to stay in her home have proven to be nonreactive and excellent with her DISABLED children. Please note: the Owner does not offer boarding to new clients.

19. Veterinary Release: The state of MT requires all clients to complete a veterinary release form.
20. In the event of an emergency, we will make every effort to contact the owner and their emergency contact listed on our way to either the veterinarian listed or the emergency veterinarian.(We utilize Sentinel Vet on S. Reserve St. (406) 218-2800) In the event no contact can be reached, we will seek appropriate medical treatment for your dog. You understand that you agree to assume full responsibility for payment and reimbursement for any and all veterinary services rendered.

By signing below, the owner states that they have read and agreed to all terms listed. This agreement will stay on file and be in effect for one year from the date signed and autorenewal annually until terminated/updated by Unleashed.
